This form is to be completed by parents/guardians and requests permission for their child(ren) to participate in OMC Teen Ministry events. Please be aware that:

This form is must be completed every year and applies to all OMC Teen ministry events.

Additional permission forms may be required for specific events that take place off site.

We request permission from parents to allow minors to follow the @OMCParishTeenMinistry Instagram page. (see below)

This registration includes communication permissions for flocknote, Groupme, and email.

A parent/guardian must be copied on all emails between minors and employees/representatives of the parish. (Youth Minister, Small group leaders)

The monthly update provides GENERAL information for the next month. Parents also added to the Sunday Night Recap flocknote group if/when your teen attends a Sunday night. The Sunday recap is specific to each week. You may unsubscribe at any time. *

Below are the various ways OMC teen ministry communicates with teens. The Groupme is a closed group with multiple leaders and a parent to monitor the group. Flocknote is used to send emails and text messages to groups, recipients can respond to the Flocknotes. Any emails sent directly to a teen will always include a parent copied. *
