Scuba VBC crews are mixed age groups. Please encourage your child to be open to making new friends! We are happy to accept buddy requests and do our best to honor them; however, they are not guaranteed. Please include the first and last name of the requested buddy.
Special Needs Information: Please identify any specific needs or challenges your child may have that require special attention by the program leaders. Please list any identified learning disabilities, classroom difficulties, medical conditions, allergies, etc.
I/We, the parent(s), give our permission for our child/ren to attend “Scuba” Vacation Bible Camp. We fully recognize that such undertaking involves an element of risk and assume and accept these risks and hazards which are incidental to such participation. We hereby hold harmless and release any and all rights of claim against the Corporation of the Catholic Archbishop of Seattle, Mary, Queen of Peace Parish, it's employees, and all members of the Faith Formation Staff (paid or volunteer), for any damage or injury our son or daughter may incur while participating in this event from August 5th thru August 9th, 2024. I hereby consent to participation by the above named camper (my son/daughter/individual under my guardianship), in the event described above.
Photography Release Form: I consent to photos taken of my child(ren) participating in Mary, Queen of Peace Parish – Vacation Bible Camp Program and activities for the purpose of sharing what we did with the parish either on our website, blog, newsletters, posters, etc. I recognize that my child(ren) will not be identified by name in any of the photos, without my further consent. If you do not wish to have your child pictured at all, please write your instructions to: vbc@mqp.org
The fee for Scuba Vacation Bible Camp is $70 per camper ($50/camper for families with a full time adult volunteer at camp). Please process your payment below. Your registration will not be processed without completing this step. Scholarships are available upon request - contact vbc@mqp.org
