Thank you for choosing to register your teen for Triple V Youth Ministry. This registration form is comprehensive and includes all the forms needed to partake in online and in-person Triple V Youth Ministry programming. You will need your teen(s) for certain parts of this registration form so please keep them close while you are filling this out this Google form.
